A Shocking Skin Lesion With Multiple Openings: What Could Cause an Appearance Like This?

A close-up medical image can sometimes leave people with more questions than answers. This particular image shows an irritated area on the upper back with numerous small openings, raised dark-looking structures, and surrounding redness and swelling. The unusual appearance can understandably look alarming.

However, it is important not to assume a diagnosis simply from a viral image. Several different skin problems can produce unusual bumps, plugs, openings, or crusted material, and some dramatic-looking images circulating online may also be edited, staged, or presented without their original medical context.

What Are Those Openings?

The clustered openings visible in the image may resemble enlarged pores or drainage openings, but a photograph cannot establish what is actually inside them.

Possible explanations for unusual openings or plugs in skin can include:

  • Severely blocked follicles or pores
  • Inflamed or infected cysts
  • Boils or abscesses
  • Accumulated keratin or other skin debris
  • Chronic inflammatory skin conditions
  • Healing wounds with crusted material
  • A foreign-body reaction

The dark material visible in or around a lesion does not automatically mean that insects, worms, or parasites are present.

Why Does the Surrounding Skin Look Inflamed?

Redness and swelling generally indicate that the skin is irritated or that an inflammatory process is occurring.

When inflammation becomes significant, the area may feel:

  • Tender or painful
  • Warm to the touch
  • Swollen
  • Sensitive when pressure is applied
  • Prone to drainage or crust formation

If an infection is involved, the inflammation can sometimes spread beyond the original lesion.
